Module 2.5: How to Create a Custom Forms for your Membership

You have the option to set up either or both of the following form types:

Membership Form: This form type can be created for each membership type that you create. This form will be completed as part of the membership purchase and upon renewal each year

Registration Form: This form type is asked once upon account creation and will not be asked again upon renewal, however it can be edited at any point. These form responses can be mapped directly to the users profile so is a great opportunity to collect emergency contact information and more.
